Accessibility statement for Formic surveys and forms

Preparation of this accessibility statement

This statement was prepared originally on the 17 October 2023. It was last reviewed on 17 January 2025.

This website was last tested in March 2024. The test was carried out by User Vision.

The following approach was taken for the accessibility audit by User Vision:

  • Automated evaluation using Accessibility Insights, PowerMapper and Wave.
  • Manual accessibility evaluation to code level using browser code inspectors, an approach that identifies issues that cannot be captured by automated tools.

Pages were also accessed using different browsers and assessed using the following assistive technology:

  • NVDA (with Firefox and Chrome) screen reader for desktop
  • VoiceOver for iPad & iPhone (iOS)
  • Talkback for Android

We also checked colour combinations using a colour contrast analyser to determine if there is enough contrast in the visual presentation.

Applicability

This accessibility statement applies to the following surveys created using Formic Fusion Online Forms platform:

  • inspection surveys
  • corporate performance surveys
  • the STEM grants form

These surveys are hosted for Education Scotland By Formic Ltd.

How accessible are Formic online forms

Formic continue to make improvements to its software, and prioritise accessibility in its development activities. Our aim is to continue to develop our solutions, aligning with best practices and to at least meet if not exceed existing accessibility guidelines with complimentary functionality. 

We want our forms to be usable by as many people as possible. You should be able to:

  • zoom in up to 200% without loss of information
  • navigate forms using just a keyboard
  • navigate forms using speech recognition software
  • listen to forms using a screen reader (including the most recent versions of JAWS, NVDA and VoiceOver)
  • access the forms using a mobile or tablet

AbilityNet has advice on making your device easier to use if you have a disability.
